Plot

In the aftermath of Kong's battle with the Kraken, Charlie stumbles into a snare trap and is captured by a trio of masked Skull Islanders, who drag him to their leader, a red-haired woman. She asks if he regrets engineering the battle between the two Titans, which has left Kong severely poisoned; he does not, since he only did so to save his father, Cap, and his friends. Blaming him for upsetting the balance of the island, she has the masked islanders tie him to a platform and suspend him over a pit, but promises to free him if he survives the next three days. As she leaves, she introduces herself as Kaia. With nothing else to do, he reflects on the dramatic events of the past few days: saving Annie, surviving the Kraken's attack on their boat, and meeting Annie's mother Irene and the host of monsters on Skull Island, including Kong. He realizes, as he hears a growl from below, that this pit may be one of the very entrances to the Hollow Earth that Cap believes exists.

In a flashback to the day after the Titan battle, Irene calls in a helicopter to airlift the injured Annie, while Mike is carried away on a stretcher. The girl pleads for her to find Charlie, and Sam agrees, but she explains that she wants to get everyone they know survived off the island before sending another search party. Cap stays behind, walking past the Kraken's severed head, while Irene muses that Skull Island is the sort of place he's spent half his life searching for. Dashing into the sea, Dog follows the departing helicopter.

Charlie concludes his reminiscing: with no other way to get past the Kraken, he and Annie stole a necklace precious to Kong to goad him into approaching the shore and the other Titan. Charlie leapt off of Dog when it seemed Kong might catch them to lighten his load. A giant wasp suddenly appears before him, only to be speared by Cap, who cuts Charlie loose and barely pulls them across the pit before the rope snaps. Before they can celebrate, however, Kaia's group captures them and ties them to a totem pole. Cap catches Charlie up on what he missed, including that Kong threw a tree at the first search-and-rescue helicopter Irene sent after returning to civilization. Charlie expresses relief that Kong is still alive, given what Kaia told him. An earthquake rocks the island, which one of the islanders blames on Charlie, to his outrage. The Hawk Monster arrives to tell Kaia that creatures from Hollow Earth, aware that Kong is poisoned, are responsible for the earthquake. At Cap's offer to help, Kaia asks another islander, Etana, to free them and bring them to Kong so they can see what they have wrought. As the queen, Kaia herself travels atop the Hawk Monster. Etana, quickly growing weary of Charlie and Cap's chatter, leads them to the entrance of Kong's sanctuary: the same waterfall that Charlie and Mike fell from while fleeing from a Galloping Crocodile. They swim underwater and emerge in a cavern where islanders on scaffolding tend to Kong's many wounds. Kaia intones that if Kong dies, something worse will take his place.

The Hawk Monster, flying over the island, stops to investigate a pit containing a bubbling, tar-like substance. Suddenly, a giant hand shoots out of the pit and pulls the bird under. Meanwhile, Annie wakes up in the hospital and is horrified to find herself back in civilization.
